I. Policy:

The LV Sportz Foundation establishes the following eligibility and application guidelines for Youth Sports Organizations or Events in Southern Nevada Eligibility and Use of our A.E.D. Loaner Program (“Program”).

II. Purpose:          

This policy establishes the necessary criteria and processes to determine who and how organizations may participate in the Program.

The LV Sportz Foundation is committed to providing loaner A.E.D.’s to local sports organizations or events at little to no cost to those deemed eligible for the purpose of expanding A.E.D. coverage at local recreational sporting fields and courts.

III. Definitions:

Automated External Defibrillator (“A.E.D.”):  A device which combines a heart monitor and defibrillator and has been approved by the United States Food and Drug Administration, is capable of recognizing the presence or absence of ventricular fibrillation or rapid ventricular tachycardia, is capable of determining without intervention by an operator, whether defibrillation should be performed, and automatically charges and prompts a user to deliver a shock when necessary.

Certified Safety Officer/Coach:  A person who is at least age 18; who has successfully completed and certified by the American Red Cross or American Heart Association in CPR and A.E.D.; Certified Safety Officer/Coach status with LVSF is good for the term of the Officer/Coaches certification as determined by the expiration on his or hers CPR/A.E.D. Certification card.

Qualified Organization: A youth sports organization within or having an event take place within Southern Nevada that needs an AED onsite and has a Certified Safety Officer/Coach.

Return Date: This date will be within 72 hours of the end of the season/event, as indicated on the Use of Equipment Agreement forms.

IV. Eligibility/Procedures/Guidelines:

A. Eligibility Criteria for Qualified Organizations:

AED’s may only be provided to Southern Nevada Youth Sports Organizations or Sporting Events who meet the following criteria:

  1. Have an assigned LVSF P.A.S.S. Liaison or a Safety Officer/Coach who is certified in CPR/A.E.D. by the American Red Cross or American Heart Association. LVSF may offer an American Red Cross Certification Class if organization has 3 or more staff members interested in obtaining certification.
  2. The organization or event must be within Southern Nevada.
  3. Agrees to Release of Liability (Form # GS-AED-2).
  4. Agrees to all Terms & Conditions outlined in the Use of Equipment Agreement (Form # GS-AED-3).

B. A.E.D. Loaner Application Process & Guidelines

  1. Qualified Organization must first submit a request via our website.
  2. LVSF will look over requests in the order received with the priority going to first request received that meets our Eligibility Requirements as a Qualified Organization.
  3. Usage of A.E.D. loaner must be at a location that is within Southern Nevada unless otherwise agreed upon by LVSF. A team or organization that resides in Southern Nevada traveling temporally out-of-state for a sports-related event may be eligible if approved by LVSF.
  4. Requests for usage of an A.E.D. loaner must be received by the LVSF no later than 2 weeks in advance of the intended usage date.
  5. Approval of requests will be subject to A.E.D. and Training Availability. Available A.E.D.’s are given out on a first come, first serve basis to ‘Qualified Organizations’.
  6. Approval or denial of the request will be communicated to the applicant by LVSF via one of the following methods: phone, electronic mail, mail, or fax.
  7. For approval of the request, the ‘Qualified Organization’ must designate/name an individual as the ‘Certified Safety Officer/Coach’ on the application. This individual will be responsible for the training, transportation, storage, operation, and return of the A.E.D.
  8. The ‘Certified Safety Officer/Coach’ for the organization will be required to (1)sign a copy of the Policy & Procedures (2) sign a release of liability waiver, Form # GS-AED-2, (3) sign an A.E.D. Equipment Use Agreement, Form # GS-AED-3.
  9. After successful approval, an AED will be issued to the ‘Qualified Organization’. ‘Qualified Organization’ status for the applicant will be in effect for the time issued.
  10. A.E.D. pick-up/return will be as agreed upon by both parties.
