  1. Release of Liability (Form # GS-AED-2).

  2. Use of Equipment Agreement (Form # GS-AED-3)


The above forms must be read before submitting request. They can be filled out and handed-in, in person, once approved. Certified Safety Officer/Coach must be present at meeting and must provide a copy of a valid ID and CPR/A.E.D. Certification Card or Certificate by the American Red Cross or American Heart Association.

Organization Type * Organization or event must be in Southern Nevada
Is the Organization or Event Within Southern Nevada? * The requesting organization or event must be within Southern Nevada
Does Your Organization Have a Safety Officer/Coach? * This officer/coach must have CPR/AED Certification from the American Red Cross or American Heart Association
Projected Start Date When Needed * Please tell us the date in which you will need to have possession of the A.E.D.. It is recommended to have the A.E.D. before your organization starts any physical activity in which would cause exertion on the heart of athletes...
Projected Return Date * Please tell us the date in which you think you will return the A.E.D.. It is recommended to have the A.E.D. as long your organization has any physical activity in which would cause exertion on the heart of athletes...
